Inhaled glucocorticoid (ICS) is currently the strongest local airway anti-inflammatory drug, which exerts an anti-inflammatory effect by affecting cells and molecules related to inflammation. Commonly used ICS are budesonide, fluticasone propionate, beclomethasone propionate, and ciclesonide. What is the difference between these four ICS?

I. The same inhalant, but different indications

Budesonide aerosol is only approved for the treatment of bronchial asthma; the use of albuterol and budesonide nebulizer to inhale budesonide powder is approved for the treatment of bronchial asthma and chronic obstructive pulmonary disease. Whether it is aerosol or dry powder inhalation, fluticasone propionate, beclomethasone propionate, and ciclesonide are only approved for the treatment of bronchial asthma. why?

1. Different lesions:

The main lesions of bronchial asthma are the bronchi, and the main lesions of chronic obstructive pulmonary disease (COPD) are the terminal bronchi and alveoli.

2. The size of atomized particles is different:

The fog particles with a particle size of 5~10μm are mainly deposited in the oropharynx, the fog particles with a particle size of 3~5μm are mainly deposited in the lungs (bronchi, bronchiole), and the fog particles with a particle diameter of <3μm are mainly deposited in the terminal bronchi and alveoli.

A nebulizer is a medical device that turns liquid medication into a mist or an aerosol so that it can be inhaled directly into the lungs through a face mask or a mouthpiece. Feellife mesh nebulizer has mini and portable size, convenient to use at anytime anywhere.

How does nebulizer work?
It transforms the liquid drug into mist and with inhalation, the drug directly enters into respiratory tracts and lungs, achieving an painless, fast and effective aerosol therapy. It was driven by the circuit rapid oscillation, Piezoelectric ceramic transducer starts resonant oscillation and generates the rapid oscillation of the mesh, through which, the liquid drugs are popped out rapidly while generating numerous small mist particles(1-5μm).

How to use Feellife portable nebulizer？
Open the medication cup lid, pour in proper liquid medication, do not exceed the max scale indication. Close the medication cup lid, connect mask or mouthpiece, put it on, then press the power button for 1-2s. It start working when you see the mist come out.

How to clean portable mesh nebulizer?
Like any other piece of medical equipment, it is important to clean and disinfect after each use. First, remove the mask, open the medication cup lid and pour out all medication residues. Then wash it with warm water(below 40℃) and rinse it 2-3 times. Cover the lid and nebulizing with warm water(below 40℃). Then wipe the remaining water with a dry cloth. Nebulization with one drop of purified water and three drops white vinegar will achieve better cleaning effect.

Note: Do not touch the central area of the mesh to avoid the damage of the mesh.

What medication is used in a nebulizer?

Why should nebulization be stopped after 10 minutes?

Antibiotics are used in atomization? Almost impossible!

Clinical Conditions:

At present, children's antibiotics commonly used in clinics have no atomized dosage forms in the market.

On the one hand, there is no safe, reliable, and effective clinical evidence. On the other hand, there is no authoritative guide to guide medication.

Therefore, at present, there is no available atomized dosage form containing antibiotics in China.

At present, there is no atomized dosage form containing antibiotics that can be used by pediatrics in China. Therefore, atomization has no direct relationship with antibiotics and abuse of antibiotics.

Don't Use These Medicines Either.

Common Antibacterial Drugs and Chinese Patent Medicines for Treating Respiratory Tract Infections in the Market

1) People use antibacterial drugs for atomization administration mainly to increase local drug concentration and reduce systemic adverse reactions. By searching the literature, it can be found that antibacterial drugs used for atomization inhalation include gentamicin, amikacin, vancomycin, and so on.

Many people like to use gentamicin for atomization, but instructions of gentamicin and pharmacopoeia do not mention that gentamicin can be used for atomization. There is still insufficient medical evidence for its efficacy and safety.

Therefore, the above-mentioned “Expert Consensus” pointed out that local application of antibacterial drugs should be avoided as much as possible. Up to now, there is no special preparation of an antibacterial drug for atomization inhalation in China.

Intravenous preparations are used to replace antibacterial drugs in clinics and in most studies. However, intravenous preparations are not completely suitable for atomization, because they contain preservatives.

2) The clinical experience and basic research of atomization inhalation of Chinese patent medicine are insufficient. The reliability and safety of the curative effect need to be verified, so it is not usually recommended.

Three Kinds of Commonly Used Drugs for Atomized Inhalation

Glucocorticoid

It is the most widely used and the most effective way to control airway inflammation. It is the preferred drug for treating bronchial asthma, various respiratory diseases related to wheezing, cough-related diseases, and various respiratory diseases with wheezing symptoms.

At present, there are three kinds of inhaled glucocorticoids commonly used in clinics, which are budesonide, beclomethasone propionate, and fluticasone propionate, with the following characteristics.

Expectorant

1) N-acetylcysteine
It is a commonly used expectorant aerosol inhalation at present, which can reduce the viscosity of sputum and facilitate sputum discharge. However, the use of this drug is controversial in the EU.

Therefore, in the consensus of experts on aerosol inhalation therapy in China, this drug can be used for the treatment of idiopathic pulmonary fibrosis. That is to say, it is usually unnecessary to use it for children's cough and pneumonia.

2) Bromosol Injection
Atomization inhalation is not recommended in the instruction manual, but there are clinical reports in China. There are special atomization dosage forms in foreign countries, but there are few related studies about it applied to children. It is not recommended for routine atomization inhalation.

3) 3% hypertonic saline:
The latest evidence at home and abroad shows that it can effectively shorten hospital stay and improve the clinical symptoms of children with acute bronchiolitis. If the child's clinical symptoms do not relieve or irritating cough occurs after 48 ~ 72 hours of use, it should be stopped in time. It is banned from bronchial asthma.

Cough variant asthma
BUD suspension should be inhaled by atomization for 0.5 ~ 1.0 mmg/time, 1~2 times/d. The treatment time is generally not less than 6~8 weeks.
The drug dose is the same as that of typical asthma.

Post-infection cough
BUD suspension should be inhaled by atomization for 0.5 ~ 1.0 mg/time. The frequency and time of use depend on the condition of the patient. The course of treatment generally does not exceed 3 weeks.
Asthma in infants (such as bronchiolitis)
Severe: BUD suspension 1.0mg/time and bronchodilator should be inhaled once /20min for 3 consecutive times according to the condition. The interval time of atomization inhalation can be gradually extended to 4, 6, 8 to 12 hours.
It is recommended to inhale bronchodilators by atomization. The dosage of terbutaline is 2.5mg/time if the weight is < 20kg and 5.0 mg/time if the weight is ≥ 20kg.

Moderate: The above combination should be used twice a day for 2 ~ 3 days.
If the patient is younger than five years old, the dose of salbutamol is 2.5mg/ time. The interval of medication depends on the severity of the disease.
Age: Asthma children under three years old and high-risk children need long-term inhalation of BUD suspension, starting with a dose of 1.0mg/d and gradually decreasing. Adjust the dose every 1~3 months until it reaches the minimum effective maintenance dose (the minimum effective maintenance dose of BUD suspension is 0.25mg /d).Ipratropium bromide: If the patient is younger than 12 years old, the dose is 0.25mg/time. It can be administered repeatedly according to the condition.

Mycoplasma pneumonia
Acute phase: If there is obvious cough and wheezing and there is obvious inflammatory reaction and atelectasis in chest X-ray film, BUD suspension should be inhaled by atomization for 0.5~1.0mg/ time combined with bronchodilator twice a day for 1~3 weeks.Acute phase: It should be combined with BUD suspension. Bronchodilator should be inhaled by atomization twice a day for 1~3 weeks.
Recovery period: If there is airway hyperresponsiveness, small airway inflammation on chest X-ray film or atelectasis is not completely recovered, BUD suspension should be inhaled by atomization for 0.5~1.0mg/d. The condition should be reviewed after 1~3 months.
Acute laryngotracheal bronchitis
In most studies, the initial dose of BUD suspension is 1.0~2.0mg, and then it should be inhaled every 1.0 mg/12 hours. There are also studies suggesting 2.0mg/ time, once /12h, up to 4 times/d.
Bronchopulmonary dysplasia
The dose and course of treatment for prevention and treatment of BPD by atomizing inhalation of BUD suspension are unclear. In some studies, BUD suspension 0.5mg/time was inhaled by atomization 2 times /d for 14 days.2.5~5.0mg of terbutaline or salbutamol should be inhaled once/ (6 ~ 8) h. Generally, it is used for children with bronchospasm in the acute clinical stage.

Compared with other ways of administration, atomized inhalation of drugs can directly act on target organs, with the advantages of quick response, good curative effect, less systemic adverse reactions, and no need for deliberate cooperation of patients. It has been widely used in clinical practice.

1. Chronic Bronchitis

For chronic bronchitis patients with obvious symptoms and poor effect of conventional treatment, atomization inhalation of drugs is a good choice, such as inhaled glucocorticoid, bronchodilators, and expectorants.

2. Chronic Obstructive Pulmonary Disease (COPD)

Atomization inhalation can be applied to acute exacerbation of COPD.

1) Bronchodilator
Atomization inhalation of short-acting β2 agonist (SABA) should be used. A short-acting muscarinic antagonist (SAMA) should be added when the effect is not good.

2) Inhaled Glucocorticoid
Inhalation of budesonide (BUD) can partially replace systemic glucocorticoid. BUD 6 ~ 8 mg /d (2 mg, twice a day or 2 mg, once /6 h) has similar efficacy to methylprednisolone (40 mg/d). The dosage and course of treatment should be adjusted according to the severity of the acute exacerbation. The course of treatment is usually 10 ~ 14 days.

3. Asthma

1) Bronchodilator
A bronchodilator can be used for an acute attack of mild and moderate asthma. Repeated inhalation of SABA can quickly reverse airflow restriction. It is recommended to take intermittent (every 20min minutes) or continuous atomization at the first hour of onset, and then take intermittent (once /4h) as needed. When the treatment effect is not good, consider the combination of SAMA atomization inhalation.

2) Inhaled Glucocorticoid
When asthma attacks or worsens, the combination of inhaled bronchodilators and inhaled glucocorticoids (2-4 times the basic dose) can replace or partially replace systemic hormones, especially for patients with gastroduodenal ulcers and diabetes. Treatment with inhaled glucocorticoids can significantly reduce asthma-related death or hospitalization.

4. Hormone Sensitive Cough

1) Cough Variant Asthma
Combined inhalation of glucocorticoid and bronchodilator atomization therapy can effectively and quickly relieve cough symptoms. BUD suspension 1.0~2.0mg/ time, twice a day.

2) Eosinophilic Bronchitis
The first choice is the inhalation of glucocorticoids in a moderate dose. BUD suspension 2.0mg/ time, twice /d, continuous application for more than 8 weeks.

3) Allergic Cough
Inhaled glucocorticoid, BUD suspension 2.0mg/ time, treatment for more than 4 weeks

5. Post-Infection Cough

1) Inhaled Glucocorticoid
When severe persistent cough affects the quality of life, consider inhaled glucocorticoids by atomization. The dosage of BUD suspension is 2.0mg/ time, 2~3 times/day.

2) Bronchodilator
It can relieve cough symptoms.

6. Bronchiectasis

1) Antibiotics
In acute exacerbation of bronchiectasis, tobramycin, gentamicin, amikacin or polymyxin can be inhaled by atomization twice a day for 7 ~ 14 days.

2)Bronchodilators and inhaled glucocorticoids can be used as needed.

7. Ventilator-Associated Pneumonia (VAP)

For VAP infected by Pseudomonas aeruginosa, it has been reported that atomized tobramycin or amikacin can improve the cure rate and reduce the time of mechanical ventilation.

For VAP infected by Acinetobacter baumannii, intravenous atomization polymyxin has a good effect.

8.Pediatric Related Respiratory Diseases

1) Airway Inflammatory Diseases
Atomization of glucocorticoid can effectively improve the condition of patients. BUD suspension can be used for inhaled glucocorticoids in children under 4 years old.

2) Wheezing In Children
Short-acting β2 agonist (SABA) and short-acting muscarinic antagonist (SAMA) is the first choice to treat patients of any age. Medication for children needs special attention to safety.

9. Perioperative Period

Inhalation of glucocorticoid can alleviate airway inflammation. For high-risk patients with airway hyperresponsiveness and decreased lung function, it is recommended to take inhaled glucocorticoids from 1 week before the operation to 3 months after the operation.

Inhalation of bronchodilators can effectively prevent bronchospasm and other perioperative airway complications. Patients with asthma and COPD are recommended to inhale β2 agonists and anticholinergic drugs until the day of operation.
