H-FABP is a low-molecular-weight cytosolic protein involved in the intracellular uptake and buffering of fatty acids in the myocardium. It is expressed primarily in the heart, but it is not 100% cardio specific since it is expressed to a lesser extent in skeletal muscle, brain, and kidneys. The H-FABP level in serum is elevated rapidly during myocardial injury, making it an ideal marker for myocardial infarction. It is a useful prognostic marker in patients with proven acute coronary syndrome. However, renal failure and skeletal muscle disease have limited diagnostic value for AMI.

Fatty heart type fatty acid binding proteins (FABPs) are small cytoplasmic proteins abundantly expressed in tissues with an active fatty acid metabolism, such as the heart and liver. Nine distinct types of the FABP family have been identified, with Heart-type FABP (H-FABP) being the most widely studied, primarily because it is abundant in the cardiomyocytes.

H-FABP concentrations typically peak at approximately 6-8 hours after symptoms of acute coronary syndrome (ACS) and return to normal within 24-30 hours. Although H-FABP has similar release kinetics to Myoglobin (i.e. rapid release & clearance), it is approximately 15-20 times more cardiac-specific, making it a vastly more productive biomarker for myocardial injury. Furthermore, the average serum/plasma value of H-FABP is much lower than with Myoglobin, which reduces the potential for false-positive results. 

Clinical Value:

1. Diagnosis of AMI: 30min~3h after the onset of AMI, plasma FABP begins to increase and returns to normal within 12h~24h, so FABP is one of the early diagnostic indicators of AMI. Its sensitivity was 78%, significantly higher than Mb and ckmb protein. Therefore, FABP is more valuable for early diagnosis of AMI than Mb and CK-MB.

2. Others: Plasma FABP can also be increased in patients with skeletal muscle injury and renal failure.
