The Clinical Applications of the HFNC Machine in ICU

1. In ICU, the high flow nasal cannula machine is suitable for patients with spontaneous breathing, acute hypoxic respiratory failure without hypercapnia, type I respiratory failure, acute respiratory failure, acute respiratory distress syndrome, cardiac surgery, tracheal intubation or before/after tracheotomy and extubation.

2. The patient uses the dedicated nasal catheter system for the HFNC machine to produce continuous positive airway pressure under the condition of continuous high-flow output from the device (that is, continuous positive airway pressure ventilation with non-invasive ventilation), which prevents alveolar collapse, increase ventilation, reduce breathing work consumption, thereby improving oxygenation.

3. For patients with open airways, the HFNC machine provides an effective airway protection strategy; for patients with NIV non-invasive ventilation, the HFNC machine effectively solves the problem that the airway cannot tolerate high-flow gas.

4. At present, the contraindication range for the application of the HFNC machine has not been clarified. However, as a partial replacement for non-invasive ventilation, the HFNC machine has a similar contraindication range. If there is no spontaneous breathing and serious internal environment disturbances, but different from non-invasive ventilation, for patients with acute respiratory failure accompanied by hypercapnia (type Ⅱ respiratory failure), the HFNC machine should be used with caution.

5. From our clinical application, the HFNC machine has no reliable effect in severe type Ⅱ respiratory failure, but it can be partially applied to patients with compensated type Ⅱ respiratory failure or mild hypercapnia. HFNC treatment can partially reduce CO2, enhance humidification, and help sputum drainage, but for COPD patients, pay attention to the setting of inhaled oxygen concentration and avoid giving high-concentration oxygen therapy.
